We thank you for the concerns you brought up and your awareness that Medication Safety is one of the main GOALS of Patient Safety. Medication errors and issues will be one of the most difficult areas to perfect, but we do hope that because of your interest to develop the Culture of Patient Safety, we are now on the right track.
