2016 CPEX 7: CMZ CARDIOLOGY UNIT

The CMZ Performance Excellence (CPEx) Rounds Team  visited the Cardiology Unit with CMZ Execom, MD Filipinas Rojo, HR Juvee Calica, AO Karren Masuhud and ND Celita Calma.

The CPEx Team was greeted by Cardiology Unit Head, Dr. Benjamin Lopez and Chief Respiratory Therapist, Joanne Suarez and their staff.

The Patient Safety discussions were done at the Cardio-Pulmonary Unit.

The Cardiology Unit staff were reoriented on the International Patient Safety Goals and how these could be adapted to the ancillary unit especially on Identifying Patients Correctly, Effective Communication, Medication Safety and on Hospital Acquired Infections. 

The Cardiology Unit head made sure that the personnel were productive in their sharing of patient safety issues as well as on best practices. Among of the main issues were Human Resource related to 1) staffing wherein changes had to be done to fit present staff complement while adjusting personnel requirements and 2) further developing competencies for the staff through staff training in Manila for special procedures.

We had a very interesting discussion with the Cardiology Unit and we are looking forward to development of this section.

CPEx 7 opened our eyes to new technology and needs as well as patient safety issues and we hope the focused group discussion could further contribute to the development of the Culture of Patient Safety in Ciudad Medical Zamboanga.

2016 CPEX 6: CMZ PULMONARY UNIT

Last April 13, 2016, the CMZ Performance Excellence (CPEx) Rounds Team with CMZ Execom, namely CEO Atty. Jhihann Natividad, MD Dr. Filipinas Rojo and HR Manager, Juvee Calica visited the CMZ Pulmonary Unit with Pulmonary Unit head, Dr. Josephine Gemarino and Chief Respiratory Therapist, Ms. Joanne Suarez, in attendance together with the staff.

The Patient Safety discussions were done at the Cardio-Pulmonary Unit.

The Pulmonary Unit staff were reoriented on the International Patient Safety Goals and how these could be adapted to the ancillary unit especially on Identifying Patients Correctly, Effective Communication, Medication Safety and on Hospital Acquired Infections. 

The Pulmonary Unit head made sure that the personnel were productive in their sharing of patient safety issues as well as on best practices.

We had a very fruitful discussion with the Pulmonary Unit and we know the CPEx 6 could contribute to the development of the Culture of Patient Safety in Ciudad Medical Zamboanga.

2016 CPEX 5: CMZ ICU

Last March 16, 2016, the CMZ Performance Excellence (CPEx) Rounds visited the Intensive Care Unit with the presence of Section head, Dr. Benjamin Lopez and ICU Head Nurse, Ms. Sabrina Ray Anova.

Quality Assurance Audit was conducted by the Nursing Director, Ms. Celita Calma, Assistant Nursing Director for Service, Ms. Kamille Alcala, Infection Control Nurse, Nurse Supervisor Marlon Endino together with Execom, MD Dr. Filipinas Rojo, AO Karren Mae Masuhud and HR Manager, Juvee Calica.

The Patient Safety discussion was held within the ICU area where we went over issues from 2015 and evaluated how they are at the present time.

The ICU staff were also reoriented on the International Patient Safety Goals and how these could be adapted to the ward especially on Identifying Patients Correctly, Effective Communication, Medication Safety, Wrong-Site, Wrong-Procedure Surgery, Hospital Acquired Infections and on Falls. 

The ICU personnel were very dynamic in their sharing of patient safety issues as well as on best practices.

The CPEx with the ICU Team was very productive.

We know the CPEx 5 for the Intensive Care Unit may help in the development of the Culture of Patient Safety in Ciudad Medical Zamboanga.

2016 CPEX 4: CMZ WARD H

Last March 4, 2016, the CMZ Performance Excellence (CPEx) Rounds visited Ward H (Medical/General Ward).

We started off our visit to Ward H with an audit of the ward based on the DOH Safe Hospital Initiatives and on the modified Quality Assurance Audit followed by the forum part at the CMZ Conference Room for the discussion on Patient Safety.

Quality Assurance Audit was conducted by the Nursing Director, Ms. Celita Calma, Assistant Nursing Director for Service, Ms. Caryl Jamaluddin together with Execom, MD Dr. Filipinas Rojo, AO Karren Mae Masuhud and HR Manager, Juvee Calica.

We moved over to the CMZ Conference Room where we went over issues from 2015 and evaluated how they are at the present time.

The Ward H staff headed by Head Nurse, Ms. Aubrey Mae Araneta Gregorio, were also reoriented on the International Patient Safety Goals and how these could be adapted to the ward especially on Identifying Patients Correctly, Effective Communication, Medication Safety and on Hospital Acquired Infections. 

The Ward H personnel were very dynamic in their sharing of patient safety issues as well as on best practices.

We had a very fruitful discussion with the Ward H Team.

We know the CPEx 4 for Ward H could contribute to the development of the Culture of Patient Safety in Ciudad Medical Zamboanga.

2016 CPEX 3: CMZ DEPARTMENT OF LABORATORY MEDICINE

The CMZ Performance Excellence (CPEx) Rounds Team visited the Department of Laboratory Medicine on February 23, 2016. 

We started off our visit to the Laboratory with the audit of their department and also based on the DOH Safe Hospital Initiatives and for the Quality Assurance Audit followed by the forum at the CMZ Conference Room for the discussion on Patient Safety.

Quality Assurance Audit was conducted by Medical Director, Dr. Filipinas Rojo, Administrative Officer Ms. Karren Mae Parrel with Building Administrator, Mr. Andrew De Leon and Ward F Head Nurse Kris Encarnacion.

 

Patient Safety Discussion at the Conference Room was attended by the Administrative Officer Ms. Karren Masuhud with Engineer Andrew De Leon and on the Nursing side, Assistant Nursing Director - Training, Mr. Jace Pellovello, Infection Control Nurse , Ms. Kamille Alcala and Mr. Kris Encarnacion. Human Resource Manager, Ms. Juvee Calica also attended the forum.

The Laboratory Department was represented by Quality Manager, Dr. Al Zamzam Abubakar and the Laboratory workforce.

We also went over issues from 2015 and evaluated how they are at the present time.

The Laboratory staff were also reoriented on the International Patient Safety Goals and how these could be adapted to the department, especially on Identifying Patients Correctly, Effective Communication and on Hospital Acquired Infections. 

We know the CPEx 3 for the Department of Laboratory Medicine  would be instrumental to help develop the Culture of Patient Safety in Ciudad Medical Zamboanga.

2016 CPEx 2: CMZ RADIOLOGY

Last February 17, 2016, the CMZ Performance Excellence (CPEx) Rounds Team visited the Radiology Department! 

We started off our visit to the Radiology Department with an audit of the Radiology area based on the DOH Safe Hospital Initiatives and on the modified Quality Assurance Audit followed by the forum part at the Ultrasound Reception for the discussion on Patient Safety.

Quality Assurance Audit was conducted by the Nursing Director, Ms. Celita Calma, Assistant Nursing Director for Service, Ms. Caryl Jamaluddin and Nurse Supervisor, Ms. Cinda Pada.

 

 

We also went over issues from 2015 and evaluated how they are at the present time.

The Radiology staff were also reoriented on the International Patient Safety Goals and how these could be adapted to the department especially on Identifying Patients Correctly, Effective Communication and on Hospital Acquired Infections. 

We know the CPEx 2 for Radiology Department would help develop the Culture of Patient Safety in Ciudad Medical Zamboanga.
